ABOUT US
Our priority is to deliver a high-quality, professional physical therapy experience to Southeast Tennessee, one that values not only exceptional clinical care, but the overall experience of every patient we serve. From your first interaction with our team to the results you achieve, we are intentional about creating an environment where patients feel supported, respected, and confident in their care.
This vision was shaped by years of seeing Polk County residents travel long distances to access physical therapy services. Polk Physical Therapy was founded in 2022 to change that, to bring skilled, comprehensive, and world-class care closer to home for residents of Polk County and the surrounding communities.
As part of this commitment, we continually invest in advanced technology and innovative treatment approaches designed to maximize each patient’s rehabilitation. Many of the tools and therapies we offer are unique to Polk Physical Therapy in our region, allowing us to deliver more precise, effective, and individualized care.
We believe in more than simply operating a clinic locally. We are committed to actively contributing to our community, being a source of pride for our region, and ensuring access to care by participating with as many insurance plans as possible. This commitment reflects our dedication to caring for our neighbors and supporting the long-term health and well-being of our community.

WHAT TO KNOW
Getting Started
Appointments with our clinic are usually initiated by an order from a physician. Although your physician will send the order, you get to choose where you want to receive services. You can request Polk Physical Therapy, and your order will be faxed to our clinic, or simply call us with the request to set up an initial evaluation. Our front desk will contact your physician as needed.
Your First Session
In your first visit, we’ll perform a comprehensive evaluation of your problem, educate you on your diagnosis or surgery, and work with you to develop an appropriate plan of care. For each session, we ask that you dress in a manner that allows for both mobility and access to the joint that you are seeking treatment for.
Before Your Visit
Once your appointment is scheduled, we’ll send you a link via email where you can fill out all your intake information. We find that new patients like having this option – everything you want us to know will automatically be placed in your chart prior to your first visit. If you don’t have email or access to a computer, you can always fill out your paperwork upon arrival.
Our Hope for You
As our patient, we want you to leave your initial session excited that you both understand your diagnosis and there is a plan to get out of pain. We understand that it’ss your decision to go where you feel most comfortable, and we will always work to make sure that Polk Physical Therapy is not just a convenience, but your best choice for rehabilitation.
